3801 is a two-story apartment building with four 2-bedroom apartments, located in Robbinsdale (Hennepin County). Nine people receive services here, so 2 people share a bedroom, the other 7 have their own bedrooms. The house is on the main street that runs through the small ‘downtown’ area of Robbinsdale, so it’s walking distance to many shops, restaurants, gym, police & fire station, etc. It is on a bus line. The building has a private outdoor courtyard with seating areas and a grill for relaxing, socializing, family visits and BBQ’s and a basketball hoop. The yard also has a small, raised bed for those with gardening interests. The house plans some in house, small group activities such as Bingo games, baking and crafts, and other small group outings in the community a couple times a month. The house is wheelchair accessible only on the lower level; the upper and basement levels are not wheelchair accessible. There is no elevator in the building. The apartment with the room opening does not have an accessible bathroom and is on the second level and is shared with one other person. The room is private. The building has a laundry room in the basement as well as locked seasonal storage. The staff is trained in multiple areas including medication administration, CPR, and positive behavior supports. Consultation provided by nurse, dietitian, and pharmacist.

Age range is from 44 – 82. Three of the individuals are men and four are women. People living here are supported by staff to participate in household and daily living activities to maintain and increase independent living skills to the best of their abilities. They are involved in their community and participate in classes, recreational activities, and social events. Most of the people living here have a craft or hobby that they can do independently to occupy their time daily and have their own interests that staff help support. Individuals are described in the mild to moderate range of disability. Most of the people living here are out of the home working or participating in employment support during the day, during the week. All people here are verbal and ambulatory. One person has some significant aging issues and has some 1:1 assistance to help with personal cares and independent living skills. All who live here are vulnerable in areas of physical, mental, and financial safety.
