Seed Home Healthcare has an opening a tour Thomas Residence house (located at 1324 Thomas Ave N, MN  55411).The home is walking distance to Minneapolis Public Transport bus line 7 which takes you to downtown and Theodore Wirth Regional park where there are summer and winter activities. We can also accommodate our clients to shopping which are short distance drive from the home.  Our program uses the 'Harm Reduction' model.  What is Harm Reduction? Harm reduction is a strategy aimed at reducing negative consequences associated with drug use. It is based on the recognition that many people throughout the world are unable or unwilling to stop using illicit drugs. It therefore aims to: 1. Keep people alive and encourage positive change in their lives2. Reduce the harms of drug3. Offer alternatives to approaches that seek to prevent or end drug use At Seed Home Healthcare, we understand that our clients have struggled with chemical dependency, mental illness and homelessness prior to joining our program. We recognize their struggles and work to find a better solution. We feel that our clients should not be kicked out of their home if drugs have affected their lives. We offer medication management, assistance with mental health, cognition or behavioral concerns, appointment set ups, community engagement, shopping, ADL assistance, health related assistance, home management, socialization and many more. Our staff are trained in dealing with challenging behaviors such as Self Injurious behaviors, Verbal /physical aggression, anxiety, depression, agitation, Elopement, Undesirable Sexual Behaviors, Property Destruction, Alcohol / Chemical Dependency and Positive Support Transition Plan. We have registered nurses available 24/7 on call. Our services are individualized, based on clients needs.
