Seven, one-level, completely accessible homes, located close to shopping malls, light rail, restaurants and theaters. The Hiawatha sites support individuals with complex medical needs and/or needing assistance managing the symptoms of their mental illness or brain injury. Placement depends on each individual home and the individuals that live there. Due to many of the home’s vulnerabilities, we typically do not accept anyone that is mobile and has a history of aggression or harm to vulnerable populations. Behaviors are assessed on a case by case basis, due to residents in the home. There is an activity calendar in each home, developed based on the interests of the individuals in the home. Each home supports four individuals, with each person having their own bedroom. These homes have all male, all female and mixed genders. The homes each have two bathrooms; one with a roll-in shower. Transportation is typically a mix of the locations van, Metro Mobility, medical transportation, etc. This site does have a wheelchair van. Each home’s staffing level is developed based on the specific needs of the individuals in the home. Staff is available 24 hours per day. Five of the seven houses operate with an Alternate Overnight Supervision License. Staff complete scheduled cares based on the persons needs during the overnight and the person also has a pendant to call for staff assistance. The overnight staff float between the neighboring homes, to complete cares. The ideal candidate would benefit from a fully accessible setting and Certified Assistant level of care.
