Chavez is home is a wheelchair accessible home with ramp, located in the prime area of St Paul. Our home is within easy access of public transit, community centers, schools, parks, banks, restaurants, malls, cinemas and other amenities. Rapha Health Care Services has a mission to provide specialized and flexible environment that would assist individuals with acquired brain injuries, mental illness and other disabilities to achieve their optimal level of independence. Our goal is to empower our clients with a feeling of self-control and focus their purpose toward a satisfying life after a brain injury or disability and we believe that quality of life, respect, freedom and dignity are critical areas of consideration in achieving this goal for each client we served. Rapha Health Care Services offers to clients home like environment, 24- hour supervision and continuous professional care relative to housing, hospitality services and personal assistance so that they can live independently, even as they require regular help with their daily activities. Our housing with services are directed towards meeting the very specific needs of adults recovering from acquired brain injuries, mental illness and other physical and intellectual disabilities. Our home offers an ideal setting for rehabilitation and recovery after injury. Our Team consist of RN/LPN and Direct Caregiver. We can assist in arranging for : • PT/OT • Speech Therapists • Physicians • Art Therapists • Vocational Rehab Consultants Programs and Services. Services are custom tailored to individual needs and challenges whether a transitional stay to long term care. Our qualified team and our strong clinical skills provide for a positive atmosphere and motivate each client to improve their cognitive, physical and social well-being. Each client is encouraged to work toward goals that maximize their independence. Individualized service plans that focus on skills development are developed for each client. The following areas are taken into consideration: • Communication Training, Time Management, Behavior and Social Skills • Comprehensive Nursing Program, Medical and Physical Management • Activities of Daily Living, Personal Organization • Community Participation • Volunteer Programs, Vocational and Educational Pursuits • Computer Skills / Cognitive Exercises • Cooking, Gardening, Outing, Orientation to the Community • Group Therapies. Admission Criteria: We welcome referrals of clients with brain injuries, Mental illness, physically and mentally challenged clients. Recent medical and psychological reports, as well as medication lists must be made available upon referral.
