Experienced Nurse and great team ready to care for you! 3Care Health offers a specialized home health care and living assistance to our clients who need individualized care due to illness, recovery, or rehabilitation. We are committed to transforming care to everyone we care for one family at a time while providing our clients with a dignified and fulfilling life. Our savage location offers a 24-hour residential home providing Assisted Living services in Savage, MN with onsite supervised care with experienced nurse. 3Care Health is dedicated to serving vulnerable adults with a wide range of cognitive challenges, developmental disabilities, acquired brain injuries, chemical dependency and/or mental health diagnoses. This includes individualized services to address the developmental, health, safety, and social needs of each individual in our care. We are currently accepting clients at our Savage location. Our clients will enjoy their own private bedroom in a shared 4-bedroom 4-bathroom house with ample natural light at a convenient location. Services: • Registered Nurse Services • Individualized Care Plans • Medication Management Services • Verbal or visual reminders to take medication and/or setting up medications • 24 Hour Supervision • Social outings • Personal Care Assistance with Daily Living • Hands-ON assistance with transfers and mobility • Coordinating or providing transportation to appointments • Housekeeping & Laundry Services • Nutritious meals and well balanced diets offered.
