This location serves 32 individuals and is comprised of four homes serving 8 individuals in each home. This location has private and shared bed rooms. There is a large recreation room where individuals enjoy music therapy,sports, movies, etc. This location also provides services during the day for individuals who have higher medical needs and are unable to attend offsite day treatment/habilitation programs. We offer onsite activities such as sensory, spa days, outings, and many other indoor activities.
Each home has a van that is used for medical appointments and outings. Families and guardians have the option to use our vans to assist with transportation of their loved one if desired and meet the the required criteria and complete training. This location is staffed with 24-hour nursing. Each house has a Program Supervisor. We also have a Building in Charge on-site 24/7. A nurse case manager is assigned to each home.

We specialize in providing sophisticated health care interventions to people with complex disabilities. Our programs provide a variety of professional and paraprofessional staff to meet the needs of the people we serve. They include direct support professional, 24 hour nursing, dietary consultation and administrative staff. The home provides 24-hour nursing services to children and adults, some with higher medical needs, with various physical and intellectual abilities

The individuals served are very passive/pleasant and would be vulnerable to any physical aggression.
We have experience in providing hospice in partnership with hospice providers.
