Upper Afton is a split level home with a deck and patio, non-fenced backyard with mature trees, on a quiet street near parks and trails, public transit, restaurants and a library. There is a ramp in front, a roll-in shower and grab bars. Available room is on the main flooe. This home serves both males and females ages 30's to 70. Individuals presenting with Quadriplegia, Cranial defect, Epilepsy, Cerebral Palsy, Personality disorder, Bipolar disorder, Parkinson's Disease and Mild intellectual disabilities. Individuals are independent with reminders and some physical assists. There is some physical/verbal aggression and Property Destruction. There are in-home Group and individual act ivies, such as arts and crafts. This site has a dedicated van. Day Program in the area is Midway Special Services. This home has double staffing, day staffing(single staffing some parts of the day) and are Awake overnight. 03/2021
