We Care Home Health Services LLC currently has one opening at our Coon Rapids Location. We provide 24 hours customized living/Assisted Living to our clients. Client will share a three bedroom ( private rooms) house with shared living space with other colleagues. We are able to accommodate 1:1 staffing if needed.Our services are individualized based on client needs.

We also manage both high acuity and medium acuity patients with varying behavioral problems. We are also equipped with staff that have experienced in both mental and CD related problems. Due to the intensive training and education, our staff are able to provide 1:1 for patients who require that level of care when the needs arises. Our patient population ages 18 to 65 of which could be male or female with mental/psychiatric , developmental disability, TBI, Brain injury or CD problems with other medical commodities like diabetes, high blood pressure, chronic pain. Our program has excellent community integration in which patients are engaged in various community activities with the staff presence, thereby making life more meaningful and enjoyable for them. Our staff are well trained in dealing with challenging behaviors such as: Self Injurious behaviors, Verbal /physical aggression, anxiety, depression, agitation, Elopement, Undesirable Sexual Behaviors, Property Destruction, Alcohol / Chemical Dependency and Positive Support Transition Plan. We have awake staff available onsite 24/7 and registered nurses available 24/7 on call. Our services are individualized, based on clients needs
