Golden Hearts Home Care Services LLC is a licensed 24 hour customized living . We are located in the city of Anoka. Currently has 2 large bedrooms.. Facility, offer medication management, assistance with mental health, Cognition or behavioral concerns; Appointment set ups, Community engagement, shopping, ADL assistance, and health related assistance, home management, socialization and many more. Our staff are well trained in dealing with challenging behaviors such as: Self Injurious behaviors, Verbal /physical aggression, anxiety, depression, agitation, Elopement, Undesirable Sexual Behaviors, Property Destruction, Alcohol / Chemical Dependency and Positive Support Transition Plan. We have awake staff available onsite 24/7 and registered nurses available 24/7 on call. Our services are individualized, based on clients needs.

We also manage both high acuity and medium acuity patients with varying behavioral problems. We are also equipped with staff that have experienced in both mental and CD related problems. Due to the intensive training and education, our staff is able to provide 1:1 for patients who require that level of care when the needs arise. Our patient population ages 18 to 65 of which could be male or female with mental/psychiatric, developmental disability, TBI, Brain injury or CD problems with other medical commodities like diabetes, high blood pressure, chronic pain. Our program has excellent community integration in which patients are engaged in various community activities with the staff presence, thereby making life more meaningful and enjoyable for them.
