AmeriGrace Home Care LLC (Hale House) is a community residential setting that provide structured person-centered individualized care to promote the most independence possible while providing person centered 24 hours customized living services to assist individuals through the journey of life. Our employees are trained to implement plan and monitor outcomes of individualized focused plan to satisfy individual needs. We have nursing staff on call and sometimes on site. Our support staff majority are license are well trained to handle different challenges. We strive to match clients with a compatible and qualified support staff, regardless of medical condition, physical ability or cognitive state. We provide medication administration and medication assistant based on individual needs. We provide special diet depending on our client need to foster generalize well-being Our bedrooms are private rooms with some basic furniture nightstand, dresser, Television, and refrigerator.
