Home is located in a very quiet and private neighborhood. Access to shopping centers and city transportation. Very Large size bedroom with easy access to bathroom and living room. The backyard is spacious and fenced in allowing for more privacy. The patio is easy to access for smokers. Our home offers an ideal setting for rehabilitation and recovery after injury. We have a great team of RN/LPN and Direct Caregiver CNA, PCA and HHA. We can assist in arranging for -PT/OT - Speech Therapists and Physicians appointments. The House is a fully handicap accessible unit with Roll-in-shower, wide doorways, and very spacious rooms with space for easier Hoyer operations. Services are custom-tailored to individual needs and challenges whether short or to long term care. The home has a RAMP and very huge bedrooms and backyard that is accessible. Home is located in a very quiet and private neighborhood. We have a nurse during the day and on-call after hours 24/7 Physical Accommodations Available, Wheel Chair access to all the rooms, Ramp, Hospital beds, and accessible bathroom with handrails as well as wide hallways and doorways. Our qualified team and our strong clinical skills provide for a positive atmosphere and motivate each client to improve their cognitive, physical and social well-being. Each client is encouraged to work toward goals that maximize their independence. Individualized service plans that focus on skills development are developed for each client. We have a team of staff who are trained in PDN, Trach care, VENT clients colostomy, ileostomy, catheters, and experience with diabetes, hypertension, major depressive d/o, GERD, tardive dyskinesia, anxiety d/o, and CVA.
