This home is one story handicap accessible rambler. Four bedrooms are located on main floor, there is one bathroom, handicap accessible located near bedrooms. A dining area, two living room areas, and kitchen. All is centrally located with an office off to one side. The home has an attached deck with a wheelchair ramp, yard is large and fenced. The home is located about 1/4 mile from the lake. North Shore Drive is a busy street and located approximately one block from the home. There are parks and picnic areas around the lake and a large park located in downtown Forest Lake.

There are currently 3 men being supported, ages are 38 - 67. All individuals have been diagnosed with ID's, all are in either profound to mild levels. The medical status of the individuals range from little or no medical issues to moderate medical issues. Medical issues include: Seizures, mild diabetes, incontinence. One individual does utilize a wheelchair for all transportation and another uses a wheelchair and walker alternately. Some challenging behaviors have occurred at this home, such as, self-injury, aggression, property destruction , elopement, and non-compliance. These behaviors have declined through the years and the individuals at this home are considered to be vulnerable to this kind of behavior. Some of the individuals that reside in the home do attend day programming during day hours M-F.
