Home has a large front and back yard. In neighborhood with family homes, located across the street from a church and there are a number of community parks, baseball fields and a school in the area.
The home is a single level home with 4 bedrooms and 2 bathrooms. One bathroom has a bathtub and the other has a large walk-in /accessible shower. The dining room and kitchen are large, along with a cozy living room.

Staffing ratio is 1:3 during non-programming hours and there is short periods of 1:1 staffing for travel and appointments. Staffing during sleep hours is 1:4, awake staff. There are 3 males currently residing in the home, age ranch of 50 - 56. They are very active in the community and Special Olympics. All attend a day training program during day hours.
Individuals in the home are Moderate to Severe ID. They also have varying degrees of MI diagnosis's and other health related conditions. Psychotropic medications are dispensed and monitored by trained staff who also provide medical treatments and therapies.
There are history's of physical aggression, verbal aggression, deliberate self-harm, and property destruction.
The individuals adaptive behaviors have been stable and require less direction in the form of physical prompting and cues.

Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers. All individuals being referred must meet Safety Net Criteria.
