CHECK US OUT: WE HAVE A WONDERFUL, UNIQUE PLACE TO LIVE: This 4 bed home sits on a wooded hill over looking a lake. It is @ 2 miles from Walker. We have awake staff on site 24 hours unless all housemates are gone to their day programs. (DAC-community jobs.) They are normally gone M-F 9a-3p. The main level is open concept common areas. We have two current options available - a large bedroom on main floor with private half bath.) This bedroom room has a large closet, Direct TV hook up available. One main bathroom with tub/shower ( handicap accessible/over head lift system) is about 8 feet from this bedroom door. We also have the option of a garden apartment with 2 full bedrooms. It has 1 bath. The main garden area is also open concept living room, dining room and kitchen. One exit goes directly to the walk out garage the other a French door on the lake side. There is a lift chair and ramp in the main stairway that our housemates use x2 daily M-F to reach this lower level to meet their rides for their day programs. The bedroom, bath areas of the garden apartment are completely private. We currently have 2 males 28-62 and one female 54. This diverse set of ages have worked wonderfully as we find the younger ones seem to increase the interest of our older housemates and they in turn seem to provide a calming life style for the younger ones. Limited drama. We are very glad to offer person centered planning and get our people involved in the community. We have our people access the local DAC, Jack Pine Stables, Special Olympics, support community employment and other activities of their choice such as: TKD, Zumba, Dances, POW-WOW's, library, we have a local fitness center. In house we have basic fitness equipment: treadmill, glider, stationary bike, yoga, small weights, and fitness tapes.

Nursing Support:
Additional Notes and Information:
We have a 1: 4 or staffing pattern which has at times increased to 1-3 as housemates needs evolve. Our medications are over seen by the coordinator and dispensed by trained direct support staff. All medical visits, and medication renewals and ordering are again over seen by the coordinator. We do not currently have nurse visits. If our housemate would require this level of care it could be provided. We are about 2 miles from 2 clinics and 20-30 minutes from the nearest hospital. We work closely with our local clinics to provide the needed medical care for our people. We have a recliner lift chair in house, a overhead lift system in the main bath, and hospital bed with alternating pressure mattress. We have walkers, and wheelchairs for temporary use if needed. Our van has a lift seat in the front that goes out so the person may sit down before entering the van. The chair is then moved back into the van. If durable equipment needs are greater than we currently have, we work with our medical providers to get the need equipment.
