This 4 person group home is located in a nice Roseville neighborhood and operates much like other similar sized SLS and ICF homes. We have experience working with people who have maladaptive behavior(s) as well as physical and medical issues. The house is all on one level and is accessible. The current group of people residing at Axis on Belmont consists of people who are ambulatory and one person who uses a wheelchair for mobility. No one expresses themselves verbally. There are two woman and one man. We have an onsite Supervisor and nurse case management.

We have nurse case management services at Axis on Belmont. We also have 24 hour access to on-call nursing. We schedule and attend appointments per the wishes of the person and their guardian. We communicate any changes in a person's condition in a timely manner to guardians, doctors, case managers.

We would need to know more about a person's physical aggression prior to accepting them to this home. We do not employ emergency use of controlled procedures.
