Phoenix is a split foyer home, with an upper and lower main living space, and each person has their own bedroom. The home has an upper wooden deck overseeing the back yard, with stairs leading down to backyard. There is a lower level door exit out to the back yard as well. It is situated on an average size lot, with a level front lawn. Majority of the back lawn is level and surrounded by a wooden fence. The home is in a residential neighborhood across the street from a small Community College and High School grounds. The city of Faribault is a smaller community where businesses, government buildings, health care, retail and grocery stores, restaurants, churches, community college, nature center, and emergency responders, etc. are all within a 10 minute drive. Public transportation is available.

There is an LPN on staff for 7 programs; and also as part of some SAs. Employees are highly qualified professionals who are dedicated to providing person centered service and individualized care. They are experienced in CPR/First Aid, Medication Administration, Positive Behavior Supports and all other trainings required to meet the rules and regulations of 245D.

None of the ladies currently residing in the program display challenging behaviors, this would be a disruption to the program and routines.
