Single family home with a full basement. The home has 4 bedrooms and two bathrooms. They are separated into two wings by a living room and dining room. One bath has a walk-in shower and the other bathroom has a bathtub. The back patio area is located directly off the kitchen. The home does have room to maneuver a Hoyer Lift and the individuals do have room to navigate safely using wheelchairs or walkers as needed. The home has off-street parking and large driveway where there is room to safely load and unload the wheelchair adaptive van.

Individuals have diagnosis of Severe to Mild ID. They also have various additional mental illnesses and other health related conditions. Psychotropic and other medications are dispensed and monitored by trained staff.
Individuals have histories of physical and verbal aggressive behaviors or have engaged in property destruction but with age these behaviors have become less frequent.

Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers. All individuals being referred must meet Safety Net Criteria.
