The home is located on the Western end of Redwood Falls. It is approximately 2 blocks from Hwy 19. The home is in a residential area and is within walking distance of Alexander Ramsey Park and several other recreational areas. Redwood Falls has a large community center where individuals can do leisure and sporting activities.
The home is handicap accessible. There are 3 bedrooms and fully accessible bathrooms, one contains a whirlpool tub. The bathroom with the whirlpool tub also has an overhead lift system.
One bedroom has it's own living room area and bathroom (currently vacant).

Currently there are two females residing in the home, ages 27-70. Diagnosis's are BI and Severe ID.
There is a history of physical aggression, elopement, deliberate self-harm, and property destruction. Staff are trained in redirection strategies.
Staff assist with ADL's and are trained in the use of Hoyer lifts and Easy Stand lifts. Staff are also trained in medication administration.
One of the individuals attends day programming full time and the other attends part time.
Two staff are on duty many times during peak hours.

Self Injurious, Verbal Aggression, Physical Aggression, Property Destruction, Elopement, Emergency Use of Controlled Procedure, Positive Support Transition Plan

Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers. All individuals being referred must meet criteria and be approved as Safety Net.
