91st Crescent is a handicap accessible home. All bedrooms are located on main level. Quiet residential area with neighboring homes close by. There are a number of families living in the area, along with regular school bus routes nearby. Home is licensed for 4 and currently there are 3 men residing in this home. This home currently provides 24 hour support and is a male only home. Ages of current individuals is 22-70. Two of the three individuals are ambulatory but do occasionally use a wheelchair.

Individuals in this home have significant intellectual disabilities and have limited communication skills. The physical and emotional needs of the individuals in the home include: seizure disorder, obsessive compulsive disorder, specialized diets, and autism. Psychotropic and all other medications are dispensed and monitored by staff. The home has one vehicle that is accessible for all of the transportation needs for the program. The individuals in this home enjoy many activities such as eating out, picnics, gardening, car shows, movies, volunteer activities, listening to music, etc.

Staffing varies based on needs of home. This home has an awake overnight staff. Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers. All individuals being referred must meet Safety Net Criteria. To complete a referral go to DHS E-Doc system and referral number is: DHS 3912
