This is a single family home with 4 bedrooms and 3 bathrooms. The home is handicap accessible and has a 2 stall garage. The home is set in the county just west of Akeley MN. There is a bike trail near by that the individuals utilize often for walks and bike rides.

There are currently 3 adult males in their late 50's to mid 60's residing in the home. All of these individual's have a diagnosis of mental illness with schizophrenia being the most common diagnosis. Psychotropic medications are administered and monitored by trained staff. Currently a psychiatrist visits the home quarterly with consultation services available 24/7. Essentia Health Care is nearby in Park Rapids as well as the VA Health System, where appropriate. There is also one LPN on staff and RN Consultant.
The individuals served in the home have long histories of failed placements in traditional community settings leading to long term inpatient stays at regional treatment centers and/or Ah Gwah Ching State Nursing home. Symptoms exhibited include auditory hallucinations, delusions, threats, verbal and physical aggression, SIB, OCD and histories of elopement. The individuals in this home do not work outside the home. Leisure activities are provided on a daily basis. Activities are geared towards each individuals personal preferences. They can range from walks, working out at a local fitness center, bowling, fishing, attending church, movies, boat rides, etc.

Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers. All individuals being referred must meet Safety Net Criteria.
