Home is licensed for 4 individuals. Single family home with attached garage. Home is on a corner lot in a fairly quiet street. Home has 4 bedrooms and 2 bathrooms, all with wheelchair accessible showers and one bathroom has a whirlpool tub with chair lift. Outside of the home is also wheelchair accessible, including a sidewalk from patio to the front of the home. Home has an open kitchen, dining room, living room, and den. The laundry room is off of the den, along with an office and medication room. Home has an unfinished basement that is used for storage.

Currently the home supports both male and female in their 50's - 70's. All individuals have been diagnosed with intellectual disabilities in the profound to moderate range. All individuals are ambulatory, with one individual using a walker. One individual uses a wheelchair for community outings and to get to and from appointments and work. The home has a wheelchair accessible ramp van with tie downs and a regular minivan. Hoyer lift is available for use and transfers if needed. All individuals attend a vocational program in the community. All individuals have limited communication skills. All are dependent on staff for ADL's. Home does have an LPN on staff due to medical issues with current individuals.

Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers.
