Lakeland I home is not handicap accessible. Rambler style home has 3 bedrooms on the main floor, with shared bathroom. 4th bedroom and bath is in walkout basement. Available bed is on main level.

All individuals currently being served are male and ages range from 56 - 75. Aggression levels are low. Two individuals are retired and home during day hours. The third individual attends a NuVisions work program.

Aggression levels are low. Individuals currently being served are mild to moderate range of ID. Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private residential community providers. All individuals being referred must meet Safety Net Criteria. To place a referral go to the DHS E-Doc system. Referral number is: DHS 3912. Must be completed on line.
