Lakeland II is not a handicap accessible home. This is a rambler style home with 4 bedrooms on the main floor. There are 2 bathrooms on the main floor and one bathroom in the basement. The back yard is large and fenced in. One of the bedrooms has a patio door and a deck.

Currently there are 3 males served at this site. They range from 50-70 years old. All have an ID diagnosis in the Mild to Moderate range and all have a secondary diagnosis such as; Prader-Willi, schizoaffective disorder or intermittent explosive disorder.

Challenging Behaviors: Self Injurious, Verbal Aggression, Physical Aggression, Elopement

There is a 1:4 during awake hour staffing ratio and 1:4 awake over night staffing ratio. Three of the individuals currently have 1:1 hours daily. Two individuals work at NuVisions and WAVES during day hours and one is retired and stays home. Community Based Services - MSOCS is a Safety Net Provider. Prior to referring an individual to CBS/MSOCS, it is expected that all individuals being considered for admission have reviewed and been turned down for alternative support and/or placement options through private community providers.
